The Truth About Radiofrequency for Dry Eye – Is It Worth It?

Dry eye disease is one of the most common conditions we manage, often presenting as gritty, stinging, watery, or fatigued eyes that can significantly affect daily comfort and quality of life. As awareness of dry eye grows, so too does the number of treatment options being marketed, including newer technologies like radiofrequency (RF) therapy.

But how effective is RF in treating dry eye, and how does it compare to more established, clinically validated options?

Let’s take a closer look.


What Is Radiofrequency (RF) Therapy?

Radiofrequency therapy uses controlled heat, delivered through high-frequency electrical currents, to warm the skin and underlying tissues. In the context of dry eye, particularly evaporative dry eye linked to Meibomian Gland Dysfunction (MGD), the goal is to soften and release thickened oils trapped in the eyelid glands, improving the quality of the tear film.

Some clinics also promote RF for its skin-tightening effects around the eyes. However, in the wrong hands or with excessive use, RF near the eyes can sometimes dehydrate the skin, leading to under-eye hollowness, puffiness, or an exacerbated appearance of eye bags, outcomes that are especially concerning for those already experiencing ocular surface sensitivity.


What Does the Evidence Say?

The TFOS DEWS III Management and Therapy Report (2025), the most comprehensive global review of dry eye therapies to date, includes a brief mention of RF as one of several in-office technologies targeting MGD. However:

  • There are no specific studies referenced regarding RF’s safety or effectiveness in managing dry eye
  • No detailed protocols, outcomes, or clinical guidance are provided in the report
  • In contrast, IPL (Intense Pulsed Light) is discussed at length, with published evidence supporting their role in reducing inflammation, improving gland function, and enhancing ocular comfort

In other words, radiofrequency remains an emerging adjunct therapy, not yet established as a proven first-line treatment for dry eye disease.

While early evidence is promising, it’s important to note that RF is still considered off-label for dry eye management. Devices like the Forma‑I are FDA-cleared for cosmetic skin tightening, not specifically for ophthalmic use. Standardised clinical protocols, such as ideal energy settings and treatment intervals, are still evolving.

Some clinics also caution that improper use or over-treatment may lead to side effects, including dryness of the delicate eyelid skin, puffiness, or increased sensitivity.
